I understand that histogram and colour adjustments are reflected in the image saved with the "Save with Adjustments" option, but not with "Save as 16 Bit Stack"; what about Gaussian blur and deconvolution?
Can deconvolution actually worsen the image, under default settings?

gaussian blur and deconvolution (and unsharp mask sharpening and all the rest) too... They are not included in the 'Save as 16 bit stack' but are included in 'Save with Adjustments' and save exactly as seen.
Convolution is a tricky one to get right - you need to keep the slider turned down for noisy images, but can increase the value as the stack becomes less noisy. If you turn it up too far you will start to see minor issues to begin with (dark rings round stars), followed by more serious image problems as it gets much too high. Because of this there is no single 'right' value for the deconvolution slider.
